  • Coordinates total plan of care with other healthcare professionals in a Point of Care setting.
  • Makes initial and comprehensive nursing evaluations, develops plans of care, and submits accurate documentation.
  • Performs patient assessments and develops plans of care.
  • Ensures patients meet home health eligibility and medical necessity guidelines.Key Requirements:
    • Current RN licensure in state of practice
    • Current CPR certification
